- Mind LA members that register for www.mindla.org and individuals that sign up to receive Mind LA e-communications orbrochures, or Mind LA’s free cognitive assessment initiatives,voluntarily provide us with contact information (such as nameand e-mail address). We may use this information for specific,limited purposes. You may always "opt out," either now or at anytime in the future, if you do not wish to receive our messages.
We only collect personally identifiable information ("PersonalInformation") that you provide to us voluntarily when you fillout a form, make a request or inquiry, make an online donation,request for referrals or register for cognitive assessment. ThePersonal Information we may collect includes the following:
Contact Information – sWhen you contact us with a
question or comment, we may collect Personal Information
that allows us to be in contact with you, such as your name,
address, email address and phone number.
Payment Information – If you make an online donation,
we will ask you to provide us with your name, address,
email address, phone number and payment information such
as your credit card number and expiration date. You may
also choose to provide us with additional information such as
your motivation for the cause, family history etc.
Registration Information – We may provide online
registration for our free cognitive assessment drive. When
you register for a cognitive assessment, we may collect your
Contact Information and other Personal Information you choose to provide to us such as your preferences and connection to the cause.
Referral Information – We may collect your Personal
Information to share with our partners providing further low
cost or free of cost diagnostic or caregiving services - in case
you opt for it.
In addition, we may collect Personal Information that you
voluntarily provide to us when you submit a survey, request or
inquiry or otherwise correspond with us, or link your social media
account (e.g., Facebook) to our website.
- How We Use Personal Information?
We use your Personal Information to include you in our free
cognitive assessment services and send you periodical
information about our outreach programs. In particular, we use:

comments, requests and inquiries and otherwise correspond
with you, including sending you Mind LA email bulletins or
news and other information we think you will find valuable.
to process your donations and
manage our donation lists.
to plan and organize the free
cognitive assessment events for which you have registered
and help us cater the event to you.

- What Personal Information Do We Disclose?
Affiliates – We may share Personal Information with any of our subsidiaries, or our ultimate holding company and its subsidiaries, for the purpose of managing our cognitive assessment initiative and improving our website and other services.
Partners – When you contact us or make an online donation to us, it's possible your contact information may be shared with other reputable organizations for their marketing purposes. As a donor, you may request to have your name removed from these mailing lists by ‘Opting Out’ (See the Opting Out section below).
Service Providers – We may share Personal Information with third-party service providers who perform a variety of functions to enable us to provide our services. These service providers only have access to the Personal Information necessary to perform their functions, only act on our behalf and under our instructions, and may not use your Personal Information for purposes other than to perform such functions.

We respect your privacy and recognize that you may wish to limit the ways in which we or our partners contact you. Simply send an email to firstname.lastname@example.org with the following information:
- Communications from Mind LA and its Partners. To remove your contact information from mailing lists shared with other organizations, please provide your full name, mailing address and a sentence requesting removal.
- Mind LA Postal Mail. To remove your contact information from the Alzheimer's Association postal mailing list, please provide your full name, mailing address and a sentence requesting suppression of your personal information in our files.
- Social Media Accounts. You may link or unlink your social media account (e.g., Facebook) by visiting the settings in the relevant social media application.
To opt out and remove your name from future marketing emails from us, follow the unsubscribe instructions in the message.
- IP Addresses
Mind LA uses your IP address to help diagnose problems with our server, to administer www.mindla.org, and for statistical metrics used to track website visitor traffic.
www.mindla.org uses "cookie" messages to automatically help provide better services. They remind us who you are and your preferences for our website based on what you've done and told us before. The "cookie" is placed in your computer and is read when you come back to our website. Cookies let us take you to the information and features you're particularly interested in.
